Interview Questions & ID Check
Digital Form or Printable Form
The first step is for the Team Leader to complete the Interview Questions and ID check with the new volunteer. Once we receive this form from the Team Leader, we will send the volunteer the background check form to complete. The Team Leader will be notified once the volunteer has been cleared to begin serving.
Church Partnership Agreement Form
This Church Partnership Document outlines the agreement between church partners and CEF.
Ministry Compliance Agreement Form
The Ministry Compliance Agreement Form must be completed annually by all volunteers and is due at the start of each school year.

GNC Curriculum

For Team Leaders
New Volunteers:
- The volunteer completes our Interest form
- A CEF staff member contacts the team leader to request a face-to-face interview with the volunteer. The team leader submits the Interview Questions and ID Check Form
- The volunteer receives an email with instructions to complete the background check
- Once the background check is complete, a CEF staff member notifies the team leader that the volunteer is approved to serve in GNC
